To be sure your chosen form/information prints entirely, please click on the printer icon in the upper far left of the open pdf file. When the PRINT box comes up, go 3/4 the way down on the left side to Page Scaling. Click on the little arrow and move the highlighted area to "Shrink to Printable Area", then on the bottom right under the Preview, click on "Okay."

Please fill out, print and mail the forms (with required registration fee, if applicable) to the appropriate chairman or registrar.  While we are proud to provide you with easily accessible, up-to-date forms, our chairmen are not yet prepared to receive registrations by e-mail.
